Live Well Chronic Disease Self-Management Program @ NPL

Live Well! Chronic Disease Self-Management Program (CDSMP)
Live Well is an evidence-based group workshop given for six weeks,
two and a half hours per session.

All sessions will be held from 10:00 am to 12:30 pm
Carnegie Room at the Main Library.

Session 3 – Sept. 6
Session 4 – Sept. 13
Session 5 – Sept. 20
Session 6 – Sept. 27


Anyone with a chronic condition (and/or their caregivers) can attend.
Chronic conditions can include diabetes, heart disease, cancer, arthritis, obesity, depression, fibromyalgia, and many many others. About half of the adults in the US are estimated to have at least one chronic condition.

Workshops are facilitated by two trained leaders. Subjects covered include:
1) techniques to deal with problems such as frustration, fatigue, pain and isolation
2) appropriate exercise for maintaining and improving strength, flexibility, and endurance
3) appropriate use of medications
4) communicating effectively with family, friends, and health professionals
5) nutrition

6) decision making
7) how to evaluate new treatments.

Live Well does not conflict with existing programs or treatment, but is designed to enhance regular treatment and disease-specific education. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.
